6/23/2023 EnchantaVaughn Approved 1.Behavior: Upon arrival, Jacaeri presented himself with a passionate tone when welcoming QMHP-C into the home. Jacaeri expressed he felt excited and ready for todayâs session because he had so much to talk about. Jacaeri expressed positive thoughts about his day. Jacaeri mother discussed with QMHP-C on how Jacaeri continues to struggle with being able to communicate effectively with her about his feelings and he will shut down or lash out. 2.Behavior: Jacaeri was observed by QMHP-C displaying withdrawn behaviors as he was getting out of his seat and playing with item on the table while discussing ways to manage his anger. 1.Intervention: QMHP-C observed Jacaeri in the living room in efforts to determine whether he required assistance. QMHP-C determined Jacaeri was on task and did not require assistance. QMHP-C provided positive gestures to encourage Jacaeri on his positive behaviors in the home. QMHP-C reminded Jacaeri of ISP objectives and what he needs to do in order to achieve them. QMHP-C reviewed examples of alternative appropriate behaviors he could engage in to stay on task without causing disruption. QMHP-C encouraged Jacaeri to resist negative behaviors in efforts to refrain from requiring to be redirected so often. QMHP-C praised Jacaeri for listening and following directions and interacting safely during each intervention. 2. Intervention: QMHP-C informed Jacaeri of replacement behaviors in efforts to decrease attention seeking behaviors the Jacaeri often is involved in. QMHP-C modeled appropriate ways for Jacaeri to get positive attention from his dad and grandmother. QMHP-C inquired of Jacaerisâ current mood. QMHP-C utilized active listening skills as Jacaeri spoke of his current mood. QMHP-C assisted Jacaeri in exploring other coping strategies that he had previously used to solve other problems. QMHP-C utilized behavior modification strategies including removing Jacaeri from the stimulant environment in efforts for him to reduce impulsive behaviors. QMHP-C encouraged Jacaeri to refrain from displaying negative behaviors towards his dad, to talk calmly, and express himself without yelling or getting upset. QMHP-C modeled proper positive behavioral expectations for the Jacaeri to administer additional layers of support and display an understanding of expectations. 1.Response: Jacaeri smiled as he saw QMHP-C's gestures which encouraged him to be active but focused on whatâs important. Jacaeri was able to process making good choices with QMHP-C and his dad. Jacaeri listed 3-4 behaviors that he was going to utilize in efforts to remain positive in the home. Jacaeri appeared to be receptive to receiving positive reinforcement from his grandmother, as demonstrated by him smile and warm inviting body language. The frequency and intensity of the Jacaeri's defiant affect decreased. Jacaeri continued to work on the psycho-educational activity. Jacaeri required additional redirection from QMHP-C to get him to focus on the assignment. Jacaeri was able to settle down and return to the activity. Jacaeri was responsive to counselorâs lesson. Jacaeri reduced impulsive behaviors after 3-4 prompts from his grandmother and dad. Jacaeri began to show greater control of his anger. Jacaeri appeared to decrease the intensity of his anger and frustration. 2.Response: Jacaeri verbalizes confident feelings about himself but recognizes areas he needs to work on as well. Jacaeri continued to interact in a productive manner with his father and counselor in hopes they would applaud and reward his behavior. Jacaeri was seen making the right choices and staying out of trouble. Jacaeri responded well to feedback and discussed some things he can do better when it comes to new challenges. Jacaeri acknowledges he needs to work on his temper and controlling his anger when he doesnât get his way.
